Coordinated Family Supports

RGR Solutions LLC provides Coordinated Family Supports (CFS, Service Code 076) — a Regional Center service. Our services are designed to strengthen natural supports, promote independence, and help adults with developmental disabilities and their families thrive together in the family home and community.
What Are Coordinated Family Supports?
Coordinated Family Supports (CFS) is a Medicaid waiver service in California, authorized through the Department of Developmental Services (DDS) and administered by regional centers. CFS is designed specifically for adults with developmental disabilities who live in the family home and need structured, ongoing support to maintain their safety, independence, and quality of life.
Unlike other service models, CFS recognizes the critical role that family caregivers play. Services are provided directly in the home and community, and are coordinated with the consumer's Regional Center service coordinator and Individual Program Plan (IPP).
How CFS Services Are Requested:
- Requested through your Regional Center service coordinator
- Part of the consumer's Individual Program Plan (IPP)
- Administered through the California Department of Developmental Services (DDS)
- A Medi-Cal Home and Community Based Services (HCBS) waiver service
How RGR Solutions Supports Consumers and Families
Every service is individualized, culturally responsive, and aligned with the consumer's IPP. Our team works closely with families and Regional Center staff to ensure consistent, high-quality support.
Person-Centered Planning
Services are designed around each consumer's Individual Program Plan (IPP), personal goals, strengths, culture, language, and preferences.
In-Home Family Support
We support adults living in the family home by helping strengthen natural supports, promote safety, and reduce barriers to independence.
Service Coordination
We coordinate with Regional Center service coordinators, community resources, and family members to ensure comprehensive and consistent support.
Skill Building
We help consumers build daily living skills, increase independence, improve communication, and develop the tools they need to participate in home and community life.
Crisis Prevention and Planning
We work proactively with families and service teams to identify risks, develop prevention strategies, and support family stability.
Family Caregiver Support
We provide guidance, resources, and ongoing support to family caregivers to help them maintain their wellbeing while supporting their loved one.
Community Integration
We help consumers connect to community resources, recreational opportunities, volunteer experiences, and social environments that promote meaningful participation.
Documentation and Reporting
RGR Solutions maintains thorough documentation in compliance with California Title 17 and DDS standards, including quarterly reports, incident documentation, and progress notes.
Who Is Eligible for CFS?
Coordinated Family Supports are designed for adults with developmental disabilities who meet all of the following criteria:
Note: CFS is not designed for consumers who require 24-hour care, skilled nursing services, or who reside in a licensed residential facility. Contact your Regional Center service coordinator to determine if CFS is appropriate for your situation.
How to Get Started
Services begin through a referral from the consumer's California Regional Center service coordinator. Here is how the process works:
Regional Center Referral
The consumer's Regional Center service coordinator initiates the referral process to RGR Solutions LLC.
Initial Contact
RGR Solutions contacts the consumer and family to discuss needs, goals, and how we can support them.
Authorization Review
We review the Regional Center's service authorization and the consumer's Individual Program Plan.
Intake and Planning
We complete intake documentation and develop an individualized support plan with the consumer and family.
Services Begin
RGR Solutions begins delivering authorized services in the family home and community.
Ongoing Monitoring
We provide ongoing support, regular check-ins, quarterly reports, and coordination with the Regional Center team.
