How Referrals Work
Coordinated Family Supports are authorized through California Regional Centers. Families interested in services should contact their Regional Center Service Coordinator to discuss eligibility and referral options.
Referral Process
Referral Received from Regional Center
A referral is submitted through the consumer's California Regional Center by their assigned service coordinator.
Initial Review
We review eligibility and service needs to confirm our program is an appropriate fit.
Contact with Consumer and Family
We reach out to the consumer and family to introduce ourselves and schedule an initial meeting.
In-Home Assessment
Our CFS Coordinator meets with the consumer and, at the consumer's choice, family and natural supports. The visit covers strengths and interests, daily living and safety needs, cultural and language needs, behavioral and health considerations, and family capacity.
CFS Service Plan Development
The CFS Coordinator writes a service plan identifying specific goals aligned with the IPP, strategies, visit schedule, cultural considerations, and roles for the consumer, family, and staff. The consumer and family review and approve the plan.
Services Begin After Authorization
Once the plan is reviewed and authorized by the Regional Center, services begin.
